Pharmacy Quality Scheme 2026/27: Build the Evidence Before the Declaration

The declaration window is not the starting line. Build the 2026/27 PQS gateway, domain evidence, training records and ownership while the work is still visible.

The next Pharmacy Quality Scheme deadline is not in February. For owners choosing to participate in England, the immediate gateway task is the Palliative and End of Life Care questionnaire, which NHSBSA says must be submitted through MYS by the end of 31 December 2026.

The declaration window opens later, but the evidence has to be built through the work. A certificate found in March cannot repair an incomplete questionnaire, an unowned action plan or a domain criterion the pharmacy only partly met.

In brief

What should pharmacy owners do for PQS 2026/27?

First decide whether the pharmacy will participate and which complete domains it can meet. Assign one owner, use the current Drug Tariff, NHSBSA and Community Pharmacy England resources, complete the PEoLC questionnaire by 31 December 2026, and build the required premises-level evidence as each task is completed. The PQS declaration runs from 1 to 26 February 2027; claimed gateway and domain evidence must be in place by 31 March 2027.

  • Treat the gateway as a dependency, not a final checklist item.
  • Track every criterion by person, pharmacy, evidence and deadline.
  • Review the complete domain before declaring it—not isolated activities.

Confirm the current scheme and scope

The Pharmacy Quality Scheme forms part of the Community Pharmacy Contractual Framework in England. Community Pharmacy England's current PQS hub says the 2026/27 scheme began on 2 June 2026, has £20 million funding and uses a gateway plus two quality domains. It also makes clear that NHS England will not publish a separate owner guidance document for this scheme.

Use the current Drug Tariff requirements, NHSBSA page, CPE hub, FAQs and linked resources together. Save the version or access date of the source used for each decision. An old local checklist can be useful for finding a question, but it cannot establish the current criterion.

This guide is about organising evidence and decisions. It does not replace the current scheme requirements or give clinical instructions for asthma, urgent supply, palliative care or Pharmacy First. Those parts must be completed by appropriately qualified people using the current sources and the pharmacy's own governance.

Build one PQS timeline

Put every deadline, evidence date and review point in one controlled plan. Assign an accountable owner and a deputy. For a group, separate central activities from premises-level evidence and name the person responsible at each pharmacy. A central spreadsheet cannot prove that the required action, training or record exists locally.

Add earlier internal dates for checking. A sensible plan does not place the first owner review on the statutory or scheme deadline. Build time for staff absence, failed assessments, incomplete SOP updates, missing local information and authorised corrections.

Connect the plan with the pharmacy's business continuity controls. If one person, login or device is unavailable during a submission window, the pharmacy should know who can act and where the evidence is held.

Control the gateway criterion

NHSBSA describes a palliative and end-of-life care gateway with three connected elements: the MYS questionnaire, the relevant NHS Profile Manager update for pharmacies that routinely stock the specified medicines and can support local access to parenteral haloperidol, and a premises-level action plan for occasions when required stock is not available.

The questionnaire is a current action, not a February task. Record who will complete it, which source information is needed, who will review the answers and how submission will be evidenced. Do not rely on a browser tab or an email reminder as the control.

The Profile Manager requirement depends on the pharmacy's actual stockholder position. CPE's current FAQs state that pharmacies which do not routinely stock the specified medicines are not required to begin stocking them merely to meet this part. They still need the required action plan and questionnaire. Verify the current wording against the scheme before acting.

Build the action plan from local, current information and keep it available at the premises. Name how the team will identify an open pharmacy that holds the relevant medicines and how it will find local access to parenteral haloperidol. Give the plan an owner and an update trigger; a list of contacts can become wrong before the next annual review.

Manage the two quality domains

The 2026/27 scheme has two domains: Medicines Optimisation and Patient Safety, and Professional Practice. CPE's scheme page says every quality criterion within a claimed domain must be met to claim that domain. A completed activity in one part does not create partial eligibility for the rest.

Build a criterion matrix with six columns: exact source requirement, pharmacy action, responsible person, completion date, evidence location and independent check. Link rather than copy source wording where possible, and date any local interpretation. Record questions early enough to use the official support route.

For the Medicines Optimisation and Patient Safety domain, the current CPE material describes respiratory/asthma learning and safer management of urgent repeat medicine supply requests and referrals. Keep the clinical learning, assessment dates, SOP work and evidence requirements separate in the tracker, then review them together at domain level.

The Professional Practice domain includes the current Pharmacy First clinical-services audit and peer-discussion requirements described by the scheme sources. Our pharmacy audit guide explains the operational difference between defining a standard, collecting evidence, acting and remeasuring. The scheme source still controls the actual sample, dates, submission and evidence.

Use the pharmacy clinical-governance framework to connect these activities with responsibility, competence, SOPs, incidents, complaints and improvement. PQS evidence should sit inside the governance system rather than becoming a parallel folder used once a year.

Control people, training and evidence

Start with the people who must meet each requirement, not a generic branch headcount. Record role, current working status, course or assessment, version, completion date, validity rule, certificate and any permitted training plan. Reconcile the list with starters, leavers, locums, long-term absence and people returning before the declaration.

CPE warns that some staff may need to repeat learning or assessment because of the validity period and their earlier completion date. Check the exact rule for each criterion. Do not infer current validity from the presence of an old certificate.

Store personalised evidence so it remains accessible at premises level and protect personal information appropriately. Name who can view, update and export the record. Our pharmacy intranet guide shows how ownership, version, applicability and review status can make controlled information dependable across branches.

Rehearse the declaration and verification

Two weeks before the internal sign-off date, run a mock declaration. Use the current MYS questions or official action-and-evidence workbook. For every answer, identify the source, reviewer and supporting evidence. Mark unresolved points as gaps; do not use a confident narrative to hide a missing criterion.

Separate evidence of activity from evidence of meeting the requirement. A meeting invitation does not prove attendance or action. A course booking is not a completed assessment. A draft SOP is not an approved procedure in use. A submitted audit dataset does not by itself prove that every connected criterion was met.

NHSBSA can compare declarations with available datasets and request pharmacy-held evidence during post-payment verification. Keep the submission, approval, source documents and evidence map together. Record any correction or query after submission and preserve the final position.

Only an authorised person should make the declaration. Give them a short decision pack: eligible pharmacy and band, gateway status, domains being claimed, outstanding work before 31 March, evidence location, financial position and risks. The declaration is an owner decision, not an administrative click.

Use PQS work after the claim

Close the cycle after 31 March. Which evidence was hard to obtain? Which training nearly missed the date? Which local plan depended on one colleague? Which SOP or audit finding revealed a wider operating weakness? Turn those issues into named improvements and test them through the year.

Review the cost and value honestly. Include staff time, learning, meetings, system administration, service changes and evidence management as well as the eventual payment. That does not reduce PQS to a profit calculation; it helps the owner plan capacity and avoid repeating preventable rush work.

Frequently asked questions

When is the PQS 2026/27 declaration window?

Community Pharmacy England and NHSBSA state that the declaration window runs from 9am on 1 February to 11.59pm on 26 February 2027. Check the live scheme page before submission in case the position changes.

What is the immediate PQS deadline in late 2026?

NHSBSA says the Palliative and End of Life Care questionnaire became available in MYS on 27 August 2026 and must be submitted by the end of 31 December 2026 to meet the gateway criterion.

Does a pharmacy have to complete both PQS domains?

CPE's current FAQs say a pharmacy does not need to achieve both domains to receive a payment. It must meet every quality criterion within any domain it claims, as well as the gateway and other applicable requirements.

Where should PQS evidence be kept?

Follow the exact current requirement for each criterion. Build an indexed premises-level evidence route with controlled access, named ownership and the required retention. Keep the declaration and evidence map together for post-payment verification.
